NDP 2015: Hired nurses for CARAGA Region

Department of Health Regional Office XIII in Caraga has posted the list of accepted nurses for 2015 Nurse Deployment Project.

This covers the provinces of Agusan del Norte, Agusan del Sur, Surigao del Norte, Surigao del Sur and Dinagat Islands. Congratulations to newly hired nurses for this project! Requirements below.

Please be informed that the Area of Assignment will be given during the ORIENTATION January 16, 2015; 8:00AM. Venue to be announced later.

Bring the following Requirements:

• NBI Clearance
• Cedula
• Photocopy (LBP ATM Card, Updated PRC license and PHIC OR for the 2015 1st Quarter contribution)
• BIR Tax Identification Number (TIN)
• Medical Certificate
• Expanding Envelope with colors corresponding to your assigned province.
