NDP 2017: List of accepted nurses, pre-deployment requirements in Region 10

The Department of Health in Region 10 has already posted the list of accepted nurses for 2017 Nurse Deployment Project (NDP). The initial list has the names of those who hurdled the exam and interview for provinces of Bukidnon, Camiguin and Misamis Oriental.


DOH-10 in Cagayan de Oro City covers the provinces of Misamis Oriental and Occidental, Lanao del Norte, Bukidnon and Camiguin.

NDP is part of Human Resource for Health (HRH) Deployment Program of the DOH. Aside from nurses, the list also contains names of accepted universal healthcare implementers, midwives, medtechs and public health associates (PHA). Check the following links:





Pre-Deployment Requirements

In addition to application documentary requirements, the following are needed in the signing of Contract of Service:
  1. NBI Clearance
  2. Medical Certificate (complied upon acceptance prior to contract signing)
    • Chest Xray, CBC, Urinalysis
    • Pregnancy Test for Women (witnessed by MHO/DMO)
    • Drug Test
  3. TIN Requirements
    • Original NSO Birth Certificate
    • Form 1902
    • Photocopy of NSO birth certificates of dependents (if applicable)
    • Photocopy of Marriage Contrace (if applicable)
  4. DBP Requirements
    • TIN verification slip/approved form 1902/BIR ID of applicant
    • Photocopy of 2 valid IDs with 3 signatures
    • DBP application form
    • 1x1 ID pictures (2pcs)
    • Php 100
  5. Philhealth Requirements
    • PMRF Form (downloaded through PHIC website)
    • Photocopy of NSO birth certificate of applicant
    • Photocopy of NSO birth certificates of dependents (if applicable)
    • Photocopy of Marriage Contrace (if applicable)




Deadline of Submission is on or before February 2, 2017 at Provincial DOH Office.

All accepted HRH shall attend the Pre-deployment Orientation conducted by the Regional Office, with the schedule detailed in the PDF file at the link above.
