Pre-Deployment Requirements
In addition to application documentary requirements, the following are needed in the signing of Contract of Service:- NBI Clearance
- Medical Certificate (complied upon acceptance prior to contract signing)
- Chest Xray, CBC, Urinalysis
- Pregnancy Test for Women (witnessed by MHO/DMO)
- Drug Test
- TIN Requirements
- Original NSO Birth Certificate
- Form 1902
- Photocopy of NSO birth certificates of dependents (if applicable)
- Photocopy of Marriage Contrace (if applicable)
- DBP Requirements
- TIN verification slip/approved form 1902/BIR ID of applicant
- Photocopy of 2 valid IDs with 3 signatures
- DBP application form
- 1x1 ID pictures (2pcs)
- Php 100
- Philhealth Requirements
- PMRF Form (downloaded through PHIC website)
- Photocopy of NSO birth certificate of applicant
- Photocopy of NSO birth certificates of dependents (if applicable)
- Photocopy of Marriage Contrace (if applicable)
Deadline of Submission is on or before February 2, 2017 at Provincial DOH Office.
All accepted HRH shall attend the Pre-deployment Orientation conducted by the Regional Office, with the schedule detailed in the PDF file at the link above.
